Patient Care:
Select to schedule to see Dr. Hatzakis for medical care, in the office, or, Telehealth remote video or telephone. Please review the “Billing” page before scheduling.
If you need to be seen more urgently, or, you have any questions, please let me know when you fill out the intake or go the “Contact” page and send a message. Once you’ve made an appointment, urgent requests will be moved to a sooner time.
Independent medical, Legal or Forensic:
Schedule an Independent Medical Exam, legal meeting, deposition or other forensic work. Contacted me for a fee sheet and terms. (Monday or a Friday are available, contact me directly)
Administrative appointment
Schedule an appointment, one time, or recurring, for administrative meetings; ie., board roles, registry, or just personal/collegial appointments. This is not for legal, forensic meetings or patient care
NOTE: Masks are still required per state mandate in all medical facilities, and this includes Dr. Hatzakis office. If you have had a fever or other respiratory illness, have been in contact with any persons who have been confirmed with COVID-19, please discuss this with Dr. Hatzakis before making an appointment. Dr. Hatzakis is Pfizer vaccinated. Everyone’s health and safety is Dr. Hatzakis primary concern.
